Two bakery workers died in agony after bosses sent them into a giant oven to carry out repairs on the cheap, a court heard yesterday. The machine should have been allowed to cool for 12 hours, but was only left for two. David Mayes and Ian Erickson were unaware of the full danger as they crawled into the oven because fans had cooled its outer reaches to 40c. Its core, however, was still at 100c. The repair was a delicate procedure in which they had to collect broken parts from along the length of a conveyor belt which carries bread trays slowly through the 75ft-long oven.

They had removed enough trays to allow them space within the rails of the belt to crawl along with it at the same speed. But within five minutes they were relaying terrified messages over their walkie-talkies, saying the oven was too hot. There was no way of reversing the belt and they were trapped on its journey through the oven. Mr Erickson, 44, was pulled out at the other end and died on the factory floor in front of horrified workmates Mr Mayes, 47, collapsed inside and was caught in the machinery. He died from 80 per cent burns and multiple fractures. The tragedy occurred because the Harvestime bakery in Leicester put productivity above safety, prosecutor Anthony Barker QC, told Leicester Crown Court. Harvestime had been told by the oven’s manufacturers that the operation would take four men 12 hours.

The company would have lost £1,120 for every hour the oven was shut down. The two men were sent into the oven just two hours after it had been baking bread at 260c and managers decided they could go in through the entrance hatch to avoid the cost of removing side panels. The company, part of the Walsall-based William Price Group, and three of its directors face huge fines after admitting their parts in the tragedy. Fresha Bakeries Ltd and Harvestime Ltd, Fresha’s trading arm, each pleaded guilty to two charges of failing to provide a safe system of work. Fresha managing director John Bridson, 53, of Hale, Cheshire, admitted two similar charges. Production director Brian Jones, 59, of Hartford, North-wich, Cheshire, pleaded guilty to a single count of the same offence. Chief engineer Dennis Masters, 44, of Mountsorrel, Leicestershire, admitted one charge of failing to take reasonable care for others at work.

Anthony Barker QC, prosecuting, said: "The oven was normally set to run at 260C. It had only been turned off for two hours and the centre was 100C when these two men went inside. "No-one, it seemed, looked at the temperature gauge at the side of the oven which would have indicated it wasn't safe to go in." Mr Erickson, had taken a radio with him and after a few minutes inside sent a panicky message saying it was too hot. It took 17 minutes - the time it took for the conveyor to pass through the oven - before other workers could help the men. The work should have been carried out in temperatures of no more than 40 degrees, but the gauges would have read 100 degrees.

‘The men were provided with a thin suit, hat and gloves, with protected knee and elbow padding. But these were last-minute thoughts.

‘Those men went in when it was hot enough to boil water, in such circumstances where serious injury or death was inevitable.’

On the day of the tragedy in May 1998, Mr Erickson, of Walsall, admitted he wanted to get the job done quickly so he could go home and watch the FA Cup Final with his two sons.

Mr Mayes, of Rushey Mead, Leicestershire, had told friends the night before: ‘I have a challenge in the morning. I am going for it – the money is good.’

Coleman Treacy QC, defending, said: ‘This was an avoidable accident. The company acknowledges it was at fault.

‘It failed in its duty to both men, and that has caused untold heartache to the families. For this we publicly apologise.’

Fresha Bakeries were fined a total of £250,000 and ordered to pay costs of £175,000.

On 7 January 2012, a scenic hot air balloon flight from Carterton, New Zealand, collided with a high-voltage power line while attempting to land, causing it to catch fire, disintegrate and crash just north of the town, killing all eleven people on board. An inquiry into the accident by the Transport Accident Investigation Commission (TAIC) concluded that the balloon pilot had made an error of judgement when contact with the power lines became imminent, trying to outclimb the power lines rather than using the rapid descent system to drop the balloon quickly to the ground below. Toxicology analysis of the balloon pilot, Lance Hopping, after the accident tested positive for tetrahydrocannabinol (THC), suggesting he may have been under the influence of cannabis at the time of the crash, which ultimately led to the error in judgement.

The crash is the deadliest ballooning disaster ever to occur in New Zealand. It was also the deadliest air disaster to occur in mainland New Zealand since the 1963 crash of New Zealand National Airways Corporation Flight 441, and the deadliest crash involving a New Zealand aircraft since the 1979 crash of Air New Zealand Flight 901 into Mount Erebus.

The head of the investigation, Detective Inspector Sean Hansen, testified that the balloon hit 33kV lines before the electrical arc sparked a fire.

This was followed by an LPG tank rupturing with the the gas adding to the fire. Police estimated the balloon rose to between 110 and 150m after it hit the wires and caught fire. Ms Still and Mr Jordaan jumped from the basket while it was about 20 metres from the ground but died at the scene from their injuries; the other nine on board were dead by the time the basket hit the ground.

Pilot Lance Hopping tried to out-climb the power lines once the balloon became caught rather than doing a "rip-out", in which the balloon was rapidly deflated. "Had he done so...it appears there would have been a significant chance that all of these lives would have been saved," That was because the balloon would have rapidly descended from the 10-12 metre-height at which it hit the power lines, rather than the 110-150 metres it climbed to before plunging.

It also recommended balloon passengers should be instructed to look out for wires or poles and alert the pilot to them, and that a second crew member specifically train in the "rip-out" system should be on board flights.

The coroner has released four photos of the Carterton hot-air balloon tragedy.

The balloon made contact with power lines that caused a bright arc of electricity to strike the balloons basket which ignited one of the four liquefied petroleum gas bottles. Electrical arcing occurred as the balloon caused a phase-to-phase short circuit, tripping the line and causing the 3800 properties supplied by the Clareville zone substation to lose power. The arcing caused one of the four liquefied petroleum gas bottles supplying the burners to rupture, and a fire subsequently started.

Two of the passengers jumped from the balloon to avoid the fire, falling 20 m (66 ft) to their deaths below. As the fire intensified, it caused the air inside the balloon to heat and force it to rise. Eventually, the conductor wire on the power line snapped, sending the balloon shooting upwards. The fire soon engulfed the whole balloon, and 150 m (490 ft) in the air, the envelope disintegrated, causing the balloon to fall towards the ground, with the wreckage landing in a field just south of Somerset Road, around 600 m (2,000 ft) east of the SH2 intersection.

The fast-moving Batman: The Ride coaster at Six Flags Over Georgia is one of the more popular attractions at the park. Most people try to stay away from the restricted areas fenced off around the ride because they don’t want to risk being hurt, but for reasons that officials still don’t know, 17-year-old Asia Leeshawn Ferguson of South Carolina hopped two six-foot fences and entered an area he wasn’t supposed to be in.

Witnesses told police that they believed the teen was trying to find a hat that he had lost on the ride. Asia wasn’t alone; he brought a friend along with him, but as the train car was going full speed at 50 miles per hour, Asia was hit. The force of the blow decapitated him. One of the theme part visitors, Kevin Bowland spoke to the Associated Press about the accident. Reportedly, he calls his death a tragedy and is surprised how he entered the restricted area so easily. Asia Leeshawn Ferguson was a 17-years-old boy from South Carolina. Reportedly, he was a part of the Church group that visited the amusement park in Georgia.

Indeed, his parents, Asia Ferguson III and Letha Ferguson were there too.

A Lancaster County man who was killed when his pickup struck a cow last month would still be alive had it not been for the truck’s defective airbag deploying violently, shooting a piece of shrapnel through his neck and spine, his family says in a lawsuit filed this week. Joel Knight, 52, died December 22nd 2015 after his 2006 Ford Ranger hit a cow on S.C. 522 as he was driving to work, authorities have said. He was wearing a seatbelt, but an attorney representing Knight’s family says the truck’s airbag – the manufacturer of which is accused in multiple cases of injury and death resulting from defective products – ruptured violently. A piece of shrapnel from the exploding device pierced Knight’s neck and spine.

The cow caused significant damage to the front of the pickup truck, but sadly the airbag which was meant to protect Joel Knight killed him.

A suit filed this week by Knight’s estate accuses Ford Motor Company and Tokyo-based Takata Corp. of cutting corners by producing, purchasing and installing airbags they knew were defective and dangerous. “They’ve been having problems, known defects and deaths since 2003 – for more than a decade,” said Drew Creech of Elrod Pope Law Firm in Rock Hill, which is representing the family. “It’s a problem they’ve known about and they made a deliberate decision not to recall.” The lawsuit claims Ford and Takata knew of multiple cases of people being injured or killed by exploding airbags, including a pregnant Malaysian woman who died in July 2014 after a wreck and fatal airbag rupture similar to the one that killed Knight. And yet, the suit alleges, Takata and Ford still “put profits ahead of safety” by continuing to produce and install the defective airbags.

Knight is believed to be the ninth person killed in the United States as a result of the defective airbags, federal regulators say. His death last month prompted Takata and Ford to expand a recall this week to include trucks made from 2004 through 2006 in the U.S. and Canada. Ford alone is recalling 391,000 Rangers made during that time.
 

Federal regulators say this latest recall will affect about 5 million vehicles, including those made by Audi, BMW, Ford, Honda, Mazda, Mercedes Benz, Saab and Volkswagen.

Alan Catterall

The tragic last moments of Alan Catterall have been described after he fell into an oven he was inspecting in 2010. The 54-year-old was carrying out cleaning work on an industrial oven used by Pyranha Mouldings to manufacture kayaks and canoes when he became trapped.

In 2015, Pyranha Mouldings was found guilty of corporate manslaughter with the blame lying with maintenance work practices. Following his death, a podcast told how he "pounded on the door" as he was "cooked alive" inside the machine. Catterall - a senior supervisor at Pyranha - was inspecting the oven which had developed a fault with its guide rail and was turned off. However, after going inside, the oven was switched back on and placed into warm-up mode by a colleague.

Mr Catterall died after he was trapped inside the industrial oven at the company's factory in Premier Point, Runcorn, Cheshire, on December 23, 2010. The father-of-three from Halewood, Liverpool, had gone inside to clean molten plastic escaping from a mould while the oven was switched off after a fault developed. But when the fault was repaired his colleague Mark Francis turned the power, causing the doors to slam shut and lock from the outside. In a cruel twist, Mr Francis was engaged to Mr Catterall's eldest daughter. During the trial at Liverpool Crown Court Andrew Thomas, QC, prosecuting, said no-one realised Mr Catterall was trapped inside the oven until smoke started seeping out and the 'horrific' circumstances of his death discovered.

A grieving widow last night insisted she doesn’t blame her future son-in-law who accidentally switched on a 280-degree industrial oven inside which her husband was horrifically burnt to death.

Alan Catterall, 54, desperately tried to escape, even using an iron crowbar in a bid to flee the rapidly-warming furnace, but nobody could hear his cries for help.

Health and safety has failed again, An emergency button should of been installed inside the oven so that people can be alerted from outside if a problem occurs..Sirens flashing lights installed outside the oven..The oven should be situated in a more secluded and quiet place so that cries for help could be heard..Quick release door locks from the inside..Camera system..Just ideas but this is a terrible tragedy..

The investigating inspector at the Health and Safety Executive, said, 'The doors were set to automatically close whenever the electrical supply was switched back on, which meant there was a high risk of someone being trapped inside.

'There had been no risk assessments and staff had not received suitable training on how to use the new ovens and there were no written instructions on cleaning and maintenance.

'If Pyranha Mouldings and the individual prosecuted over Alan's death had properly considered the risks to employees when they designed, installed and operated the ovens then he would still be here today.' The firm’s technical director Peter Mackereth, 60, from Llangollen, North Wales was unanimously found guilty of two health and safety breaches.

A man who guzzled up to three litres of cola every day, drank himself to death.

An inquest heard how 30-year-old Paul Inman, who had Asperger’s syndrome, would walk to the shops more than once a day to buy his fizzy drinks.

 

The huge quantities of cola, other pop and water caused his lungs to swell to four times their normal weight. He died in his sleep at a care home in Haworth, West Yorkshire. Pathologist Dr Deirdre McKenna put his death down to his excessive drinking, which began when he was 10-years-old. He was suffering from a massive pulmonary oedema which meant the large amount of fluid in his lungs restricted his breathing. This was caused by Mr Inman’s non-medical condition of polydipsia, which is Greek for “much thirst”, which in turn was a result of his Asperger’s.

His mum Alison told the coroner yesterday: “I’m not a pathologist but I made that decision two hours after he died. "I’ve said the cause of it was he drank excessively. He had done since he was 10. We used to say he had a self-destruct button.” The inquest also heard how Mr Inman never stayed still – he could wear out two pairs of trainers in a week. Care staff had to keep his cigarettes so he would not smoke 20 in one hour. When he was 17 Mr Inman was diagnosed with schizophrenia – but when his case was reviewed in 2008 doctors diagnosed that he suffered from Asperger’s, a form of autism. Bradford coroner Dr Dominic Bell, recorded a verdict that Mr Inman died of natural causes.

The 22-year-old Utah man was killed while attempting to swing from a large rock arch made popular by YouTube has been pictured. Kyle Lee Stocking, of West Jordan, died about 3:30 p.m. on Sunday, after hitting the ground below the 140-foot-tall Corona Arch near Moab according to Grand County sheriff's officials. Sheriff's Lt. Kim Neal told The Salt Lake Tribune the length of rope from the arch was miscalculated and sent Stocking swinging into the ground as he went under the red sandstone formation.

Rescuers say he was dead at the scene. He was with five friends at the time of his death.

‘It is the most tragic experience to receive the phone call from the sheriff letting you know that your son has been killed in a terrible accident that could have been avoided. It is the most awful experience a parent could go through,’ Stocking's parents, Mike and Linda Stocking, said in a statement obtained by the Deseret News.

'Pendulum' swinging is a relatively new form of recreation in Utah's canyon lands, which see plenty of injuries and deaths from rock climbing and BASE jumping, which involves leaping from a fixed object with a parachute.

On March 13, another man, Zachery Taylor, was killed rappelling at Tear Drop Arch in Utah's Monument Valley.

It's part of the recreational 'craziness' sweeping the Moab area, where the annual Jeep Safari week got started Saturday, another potentially dangerous activity that involves rock crawling in modified vehicles, said John Weisheit, of Living Rivers, a local environmental group.

'People aren't accepting nature for what it is. They have to put an element of excitement into it,' said Weisheit, a longtime rafting guide. 'People see it on YouTube and then say, "That looks like fun."'

Twin teenage brothers have died after they apparently broke into Calgary’s Winter Olympics site in the middle of the night and took their own toboggan onto the bobsled track.

Six other teenagers were injured when the toboggan hit a gate separating the luge and bobsled tracks, police said.

“It’s believed at least eight teenagers, all males, entered the property after hours and used a personal sled or toboggan in an attempt to go down the bobsled track,” Sgt Paul Wyatt said.

It is not known how fast the group were travelling at the time of the crash, but according to the website of the company that runs the park, people can go down the Olympic track with a professional pilot at speeds of up to 100 kph.

The bobsled track in Calgary hosted the Olympic competitions at the 1988 Winter Games.

At turn 5, the teens struck a large track switching element that had been used to configure the track for Luge. The impact with the track switch and the chains holding it in place resulted in death for two of the teens, and serious injuries to the other 6. Provincial court Judge Margaret Keelaghan found the deaths of 17-year-old twins Evan and Jordan Caldwell were accidental and caused by blunt-force head and neck trauma.

Police say one survivor in a deadly sledding accident at Canada Olympic Park last month tried desperately to stop the third and final sled of teenagers from going down the bobsled and luge track.

He scrambled back up the hill but he was too late.

Down went the third sled and final two boys early the morning of Feb. 6 — unaware — catching the barrier and skidding to a halt where the previous waves of casualties lay.

According to police, the Caldwell boys and six others climbed a seven-foot locked fence with three plastic sleds in-hand to get into the northwest Calgary facilities, just after 1 a.m. They headed down the bobsled and luge track in three groups. The first sled, with three boys including Evan aboard, struck the barrier that configures the track for either bobsled and luge events and sent its occupants tumbling. With no idea of what had just happened, the second sled, also with three boys aboard, started down, missing the barrier but striking one of the boys from the first group, again throwing everyone off the sled. The remaining two boys, including Jordan, were aboard the final sled — likewise with no idea what occurred and ready to go. Police say it was one of the boys from the second sled who’d tried to make his way back up in a failed attempt to warn them. The third sled took off down the track and Jordan, at the front of the sled, caught a chain on the barrier. One of the boys called 911.

Mossdale Caverns is a cave system in the Yorkshire Dales, England. It is about 5 kilometres (3.1 mi) north of Grassington, and east of Conistone, where Mossdale Beck sinks at the base of Mossdale Scar. It lies at an altitude of 425 metres (1,394 ft) on the eastern flank of Wharfedale, and extends south-east beneath Grassington Moor. The cave system is formed within the Middle Limestone of the Yoredale Series. The large stream (average flow 100 litres/second) has been shown by dye testing to resurge at Black Keld, some 4.4 kilometres (2.7 mi) to the WNW 225 metres (738 ft) lower.

On the morning of June 24th 1967 a group of ten cavers met in the Cafe Royal in Grassington Main Square (now called Annie & Betty’s) to discuss their imminent visit to the far reaches of Mossdale Caverns.

 

The group were aware of the weather forecast for that day: thunder storms for late in the afternoon, expected mainly in the south of the country. That weekend was preceded by a prolonged dry spell of weather in the Yorkshire Dales and the moor tops were quite dried out. Up to that point, June was considered to have been a dry month. 

The general opinion amongst the six very experienced cavers, who were to go to the end of the known cave (Far Stream End Caverns), was that it would be safe to do so. The expectation was that if it did rain, they knew of safe places within the cave where they could sit out any flood waters. The remaining four cavers only intended to go as far as Rough Chamber and return to the surface.

Shortly after the party entered the cave they split into two parties, six in one and four in the other. The six cavers in the first party weren’t seen alive again. No one knows what happened after the parties separated, the rest is speculation.
What is known, from subsequent visits to the blockage at the end of Far Stream End Caverns, is that the party reached there safely and carried out a small explosion in an attempt to push further into the cave. The group were well aware of the need to leave the cave and not to return to view the results of the explosion, and it is safe to assume that they began their journey out immediately after detonating the explosives. Four of the team exited safely later in the day after a shorter trip but the remaining six continued into the cave whilst outside the weather deteriorated and Mossdale Beck turned into a raging torrent, completely flooding the entrance series and much of the cave with tragic results.

The six are still entombed in the pothole, the coroner having ruled the cave should be sealed. Three years later friends moved their bodies to be together at a location called Sanctuary.

By 1am the cave entrance was under four feet of water, a 133 yard long by eight foot deep trench had been dug and a small dam but rescuers could still not enter until 11.40.

Almost six hours later five bodies were found but with the weather worsening, the cave had to be evacuated. The weather continued foul and it wasn't until 1pm on Tuesday it could be resumed. Fourteen hours later the body of John Ogden was found in a small passage.

No one had been in Mossdale before. Thoughts inexorably slipped to where the shaky dam outside was keeping them alive. But as the water slowly sank, they were joined by Tony Waltham, a London-based Wanderer who had rushed to the cave, astonished to find that no one else there had ever been to its end. At last they began the hardest journey of their lives.

Though still dankly dripping, Far Marathon now was abnormally dry. With no running water the silence was uncanny. To the leader of countless underground rescues it seemed like the silence of the grave. The 900ft crawl – 10 inches high and two feet wide – was too tight for passing if they met survivors. Eyre elected to let Waltham lead a few men forward but, frantic for their comrades, others pushed past too.

Waltham found the first two bodies jamming the passage. It was a terrifying moment; a nightmare to crawl over them. Just beyond he found three more. Of the sixth, there was no sign. It was a terrifying place. When Eyre caught up many of the team were weeping; several were retching; only Waltham spoke: "Go back, Jim. They're dead."

Mossdale caverns is a very dangerous cave to explore, Flash flooding is a real problem and if you are in a tight spot you will drown.. Is it really worth it, for some yes but you wouldn't find me doing this..

The party divided at the cave entrance. Adamson's team sprinted off with the explosives, Ogden ribbing Cunningham for using girls as a pretence for chickening out. Following with the group of four, Morag watched fiancé David disappear into the gloom.

The four emerged at 5pm. The drops of rain seemed insignificant. Cunningham and Shepherd drove Collette back to Ingleton. Adamson's six were not due before midnight. Morag sheltered in a barn. The rain became torrential. Checking back twice, she was alarmed by how fast Mossdale Beck was rising.

One inch of rain fell in three hours. When she returned at 9pm, shockingly – incredibly – a lake of swirling water covered the cave entrance. She ran desperately two miles to Yarnbury Farm where farmer Riley phoned the police. By 11.10pm the big cave rescue was on.

Three members of the Nelons, a Gospel Music Association Hall of Fame quartet and the chairman of the board overseeing the Georgia Department of Corrections, were among seven people killed in a plane crash in Wyoming, according to a statement by a group member who was not aboard the aircraft. The Pilatus PC-12/47E turboprop, a single-engine aircraft, had reported an 'autopilot malfunction during its flight' before the plane went down and the accident occurred on Friday afternoon  July 26th 2024 in northeast Wyoming.

The group took off from Nebraska and was heading to Billings, Montana, went down near the Montana border in northern Campbell County.

Local authorities reported that debris was spread over a wide area, which supports the hypothesis of a mid-air breakup.

The crash also sparked a major fire due to the plane's fuel, which firefighters tackled both from the air and on the ground.

The plane had reported an equipment malfunction that is typically associated with issues in that model, according to Keith Holloway, a spokesperson for the National Transportation Safety Board (NTSB).

Flight data reveals that the aircraft underwent severe up-and-down pitching before descending rapidly from an altitude of about 26,000 feet.

Footage of the plane plunging while in a flat spin horrified people around the world, and the cause of the accident has yet to be determined. The doomed aircraft smashed into the backyard of a home inside a gated community and transformed into a fiery wreck. All 62 people aboard were killed, among them eight doctors, according to a statement from Parana’s Medical Council. Risso and at least one colleague were headed for an oncology conference to sharpen their knowledge about a disease that kills tens of thousands of Brazilians every year. Both the plane’s “black boxes” — one with flight data and the other with cockpit audio — were recovered. The air force’s center for the investigation and prevention of air accidents began analyzing them at its laboratory in the nation’s capital, Brasilia. Airports Minister Silvio Costa Filho said the center was also opening a criminal probe. The airline Voepass and the French-Italian ATR manufacturer are assisting investigations, they said in statements.

On August 9, 2024, the Voepass ATR 72-500 twin-engine turboprop aircraft was operating flight 2283 from Cascavel Airport (CAC) to São Paulo Guarulos International Airport (GRU) when suddenly the aircraft came down in the town of Vinhedo. The plane dropped a staggering 17,000 feet in less that a minute.. The ATR 72-500 is believed to have experienced a flat spin, which occurs after both wings of a plane are stalled. Police are treating the case as involuntary manslaughter and confirmed several people have been arrested.  The victim was only known by her surname, Wu, and that her hands had become disfigured trying to escape the elevator.. The general consensus is that people can survive for around three days without water, with estimates typically ranging from two days to a week. The horrific death sparked protests in the Gaoling District of the city, with hundreds  of residents furious over the lax building management that contributed to the woman’s death.

BEIJING:A Chinese woman died after being trapped in an elevator for a month because maintenance workers shut off the power supply after failing to properly check if anyone was inside, state media reported.. Workers came to repair a malfunctioning elevator in the northern city of Xi'an on January 30, The crew found the elevator stuck between the 10th and 11th floors, shouted to ask if anyone was there, and cut the power, it said. The government said the investigation found the death was due to a grave error by the maintenance company for not opening the car before cutting the power, and not verifying after the power was off whether there was anyone inside. The deceased woman, 43, reportedly lived alone in building. China is prone to safety accidents as regulations and standards are often flouted and enforcement is lax, sometimes due to corruption. Police investigating the incident found no evidence of trauma and have ruled out homicide,

Residents of the apartment building described the incident as "horrific."

People are also questioning how the woman could have gone missing for a month without anyone raising suspicion. Police said that the deceased woman lived alone in the building and has been suffering from a mental illness since her divorce with barely any visitors.

A church organist was found dead at his Teesside home naked inside a giant plastic bag.

Ian Kemp, 48, from Stockton, was found alone in his house bound by his wrists and shins, an inquest heard.

A vacuum cleaner was also connected to the bag and it is believed Mr Kemp died after the machine had sucked all the air out of the bag. The inquest heard there were no suspicious circumstances but Teesside Coroner Michael Sheffield said such incidents were sometimes connected to sexual gratification. Mr Kemp, the organist at Hardwick Baptist Church, was found by family friend Vivienne Brand on January 26. He had lived alone since the death of his mother.

Detective Inspector David Turnbull of Cleveland Police told the inquest he was called to the house in Evenwood Close at 10.40pm that night. He said: "I saw the body of a small naked man lying inside a large plastic bag in the foetal position." DI Turnbull said Mr Kemp's legs were tied at the shins by brown parcel tape and his wrists were tied together by a silver chain. Teesside Coroners Court heard the vacuum cleaner was attached to the plastic bag and was switched on when the body was found. Det Insp Turnbull added: "There was a roll of parcel tape nearby." He described how Mr Kemp's friend switched off the vacuum and the body was taken to North Tees University Hospital.

Home Office pathologist Dr Peter Cooper gave the cause of death as suffocation. He said: "I do not see how he could have been forced into that position." Toxicology tests showed Mr Kemp had not taken any alcohol or drugs. Teesside Coroner Michael Sheffield said: "Mr Kemp was well thought of and a well-liked man. He was involved with the Baptist Church. "It was clear that Ian Kemp placed himself inside the bag, using the chains and the binding on his legs. "The vacuum cleaner sucked the air out of the bag and he died of suffocation. There were no signs he intended to end his life. "It can be done for some form of sexual gratification but it's not clear what Mr Kemp's intentions were."

Mr Sheffield recorded an open verdict. The inquest heard that evidence of identification had been provided by Mr Kemp's cousin, Melville Chisnall. Philip Tait, Minister at Hardwick Baptist Church, today paid tribute to Mr Kemp, who had been a member of the church for 26 years. "He was a friendly man, a kindly man, always ready to give a helping hand," he said. "He never married, and cared for his elderly mother for some years - if he felt that to be a burden, he never said so to me." The minister said Mr Kemp's greatest contribution to the life of the church was through his music.

"He was a self-taught amateur, but one of the best church organists I have ever heard," he said.

"What makes a good organist is not what he does in the hymns, but what he plays and how he plays when there is no set music. On that scale, Ian scored very highly indeed. "He would play exactly the right piece, relevant to the service and appropriate to the point in the service. After the service he used to go on playing, which he loved to do. Or his friends would gather round the organ to speak with him." Mr Kemp's organ is now kept in the church and used in services as a way of remembering him, he added.

Joyce Carol Vincent (19 October 1965 – December 2003) was an English woman whose death went unnoticed for more than two years as her corpse lay undiscovered at her bedsit in north London. Prior to her death, she had cut off nearly all contact with those who knew her. She resigned from her job in 2001, and moved into a shelter for victims of domestic abuse. Around the same time, she began to reduce contact with friends and family. She died sometime in December 2003. Her remains were discovered on 25 January 2006, with the cause of death believed to be either an asthma attack or complications from a recent peptic ulcer.

I hope everyone is found quickly if they die so that they can be put to rest with dignity..People should check more on others to ensure they are safe and well..Especially old people..People with illness.. Vincent's life and death were the topic of Dreams of a Life, a 2011 docudrama film. The film and Vincent's life inspired musician Steven Wilson's album Hand. Cannot. Erase. In February 2003, Vincent was moved into the bedsit flat above Wood Green Shopping City where she later died. The flat was owned by the Metropolitan Housing Trust and was used to house victims of abuse. In November 2003, after vomiting blood, she was hospitalised at North Middlesex Hospital for two days, due to a peptic ulcer.

Her remains were described as "mostly skeletal" according to the pathologist, and she was lying on her back, next to a shopping bag, surrounded by Christmas presents she had wrapped but never delivered. Neighbours had assumed the flat was unoccupied, and the odour of decomposing body tissue was attributed to nearby waste bins. The flat's windows did not allow direct sight to the inside. It was a noisy building which may explain why no one questioned the constant noise from the television, which remained turned on until she was discovered. Half of her rent was being automatically paid to Metropolitan Housing Trust by benefits agencies, leading officials to believe that she was still alive. With over two years' worth of unpaid rent totalling £2,400 that had accrued, housing officials decided to repossess the property. Her corpse was discovered on 25 January 2006 when bailiffs had forced entry into the flat. The television and heating were still running due to debt forgiveness and her bills being continually paid through automatic debit.

The Metropolitan Housing Trust said that due to housing benefits covering the costs of rent for some period after Vincent's death, arrears had not been realised until much later. The Trust also said that no concerns were raised by neighbours or visitors at any time during the two years between her death and the discovery of her body.

Vincent's remains were too badly decomposed to conduct a full post-mortem, and she had to be identified through dental records. Police ruled death by natural causes as there was nothing to suggest foul play. The front door was double locked and there was no sign of a break-in. Her sisters had hired a private detective to look for her and contacted the Salvation Army, but these attempts proved unsuccessful. The detective found the previous address where Vincent had been living, and the family wrote letters to her there, receiving no response. As a result, the family concluded she had deliberately broken ties with them.

On September 10, 2017, Kenneka Jenkins (May 27, 1998 – September 9, 2017) a 19-year-old from Chicago, Illinois, was found dead inside a latched freezer of the Crowne Plaza Chicago O'Hare hotel in Rosemont, Illinois, after attending a party there the prior day. She went to the hotel in September of 2017 to attend a late-night party on the ninth floor. But she never came home — and after nearly 24 hours of uncertainty, her body was found in a freezer in a commercial kitchen space. The cause of death was determined to be hypothermia.

Tereasa Martin is the mother of Kenneka and has this to say, the hotel and security staff didn't review video footage from in-house cameras quickly enough to help find her daughter, who wound up in an unused freezer in what her legal team described as an under-construction kitchen. She also said staff failed to intervene in the party on the ninth floor, Room 926 of the hotel , which had sparked numerous complaints from hotel visitors.

Martin's attorneys said in a statement. "Hotel surveillance videos reveal that she had walked downstairs in an obviously disoriented state and was seen by multiple staff members as she was walking toward the freezer, though no one stopped her."

The temperature inside the freezer was found to be 34 °F (1 °C) approximately two hours after the doors had been left open.

A hotel staffer who walked into the freezer around 10:30 p.m. Saturday night somehow didn't see Jenkins inside, according to Martin's attorneys, citing video footage. The young woman's body wasn't found until some two hours later. The Cook County medical examiner's office ruled Jenkins' death an accident, adding that intoxication from alcohol and topiramate, a drug used to treat epilepsy and migraine headaches, "were significant contributing factors in her death." The autopsy report noted that the freezer door had a mechanism to open the door from the inside; it did not offer an explanation for why the young woman would not have been able to get out of the deadly cold. Martin's legal team has said they believe someone outside of the freezer secured the door — which, they say, should have been locked in the first place — without knowing Jenkins was inside.

Jenkins arrived at the hotel at 1:13 a.m. Saturday. The final footage which captured Jenkins alive was time-stamped 3:32 a.m. It showed her arriving at the kitchen, where she rounded a corner towards the freezer. Several witnesses reported seeing her drink cognac, but did not see her partake of any marijuana or other drugs. Another witness reported that Jenkins "wasn't acting like her usual self", noting that she would dance a little, but later appeared to be sad and went to go sit down. So if Jenkins entered the freezer at 3:32 a.m. Saturday, and was found in the hotel's freezer by an employee at 12:25 a.m. Sunday, and was pronounced dead at 12:48 a.m. meaning she was inside the freezer for 21 hours..

The freezer, which was on and working, was described as a walk-in freezer within a walk-in cooler, and was part of an unused kitchen. Lights were apparently turned off in both chambers when she entered them. Questions remained as to why the freezer was turned on, although it was reportedly being leased out to a restaurant using space in the hotel.

Attorney Geoffrey Fieger presented blown up photographs of the freezer door, alleging it was inadvertently locked from the outside after Kenneka Jenkins went in.

1988 Seoul Olympics

During the opening ceremony of the 1988 Summer Olympics in Seoul, South Korea, hundreds of live doves were released as a symbol of world peace. Many of the doves landed on the Olympic cauldron just prior to it being lit. When the cauldron was lit, over a dozen of the doves resting on the rim of the cauldron and flying directly above it were burned alive by the Olympic flame. The death of the birds marked the last time that live doves were used in the ceremonies.

Luciano Re Cecconi 1st December 1948 – 18th January 1977) was an Italian footballer, who played as a midfielder. A fast, strong, athletic, and hardworking player, known for his speed, tenacity, and stamina, he functioned as a box-to-box midfielder or as a defensive or central midfielder for his teams. He was also known for his sense of humour throughout his career, and was nicknamed l'Angelo Biondo (lit. 'The Blond Angel'), for the colour of his hair.

On January 18, 1977, Lazio star Luciano Re Cecconi paid a fatal price when his and troublesome team-mate Pietro Ghedin's robbery prank on a jewellery shop went disastrously wrong. Re Cecconi was a key member of Lazio's historic 1974 Scudetto winning side, who went down in folklore for playing some of the best attacking football Italy's top flight had ever seen.

However, Tommaso Maestrelli’s men also gained a reputation off the field for their practical jokes and extreme right-wing political sympathies.

Several players were known for carrying guns on them wherever they went and even firing them for no reason, which created a divide between them and other disapproving players in the dressing room. On a harrowing winter's evening in January 1977 L'Angelo Biondo (The Blond Angel) went one practical joke too far when he made his way to a jewellers in Rome owned by Bruno Tabochini with Ghedin.

Re Cecconi and Ghedin were regarded by most as a troublesome duo who never failed to conjure up mischief when in each other's company, and it was no different when they visited Tabochini's shop.

As they approached the jewellers, the pair decided to hang back while their friend entered the store as part of their latest joke. The door then opened and both players sparked into life, sprinting into the shop and reportedly shouting "Hands in the air! This is a stick up!" while covering their faces with their jackets. 

Yet, what Re Cecconi and Ghedin were unaware of at the time was the fact that Tabochini's shop had been raided by burglars just a few weeks earlier, meaning he was suitably prepared for another potential break-in.

When the jeweller pulled out a shotgun, Ghedin quickly raised his hands in an attempt to diffuse the situation, but Re Cecconi failed to realise and continued with the prank. Tabochini therefore opened fire and shot the midfielder in the chest from close range, which killed him around 30 minutes after the incident despite being rushed to hospital. Re Cecconi died aged 28 and left behind his wife and two children. As he lay on the floor dying, it is believed his last words were: "It's a joke! It's just a joke!" Tabocchini was arrested but never charged or convicted of murder. It remains one of the most unusual deaths in Italian football history.

Kirwan rushed over to the pool to save the terrified dog. Some onlookers advised him against taking a step further, but he was adamant. He took two steps into the pool before diving headfirst into the boiling spring.

He was overcome by his emotions; by the time he realized the futility of his decision, he had been burned. David was dead in less than 24 hours, and the remains of his dog were never recovered. When Kirwan tried to climb out of the spring, Ratliff had to step in to rescue his buddy from the water, causing him to get second degree burns to his feet. Onlookers carried Kirwan to a nearby open space, where they attempted to console him until the ambulance showed up. Witnesses heard him say: "That was stupid. How bad am I? That was a stupid thing I did." After his rescue, Kirwan appeared to be in a terrible state. He had gone blind and had turned white, and his hair was falling out. When a park visitor attempted to remove one of his shoes, the skin came off with it as it had already started to peel off all over his body. Kirwan suffered third-degree burns on 100% of his body. Despite spending several agonizing hours, Kirwan passed away the following morning at a Salt Lake City hospital. 

In 1981, David Alan Kirwan, 24, and Ronald Ratliff were walking through Yellowstone Park with Ratliff's dog, a mixed-breed boxer named Moosie, when the pet dived into a hot spring. This proved to be a fatal mistake as the Celestine Spring that he had dived into was known to reach temperatures of 93C (200F).

A frustrated golfer who smashed his club into his motorized cart on the 13th hole died of injuries suffered from a flying piece of the club. Mike Scaglione, 26, of Chalmette, became frustrated with his game and smashed his club into the golfing cart..The club broke and boomeranged into Scaglione's neck, severing his jugular vein. The insurance man was recovering well Monday and Tuesday, the friend said, but on Tuesday night he lapsed into a coma.

Margherita Lega plunged 500ft to her death from a cable car after she was dragged away by the mountain lift system in front of her horrified family. Teacher Margherita Lega, 41, had been hiking with her husband Rocco Meloni, and their two young children aged six and nine, during a holiday when the freak tragedy happened. She had been loading luggage onto the teleferica machine - a small cable car used to transport baggage and objects rather than people - when a buckle on her backpack was snagged by the contraption and it suddenly switched on. Frantically clinging on, Margherita was carried more than 50 yards over a cliff edge, before she slipped off and plunged to her death at Calasca Castiglione near Turin, northeast Italy.

More than 100 emergency personnel including firefighters, police and mountain rescue teams rushed to the scene.

With the help of a helicopter and climbers on the ground, they eventually located Ms Lega's body in the valley.

Alpine rescue crewmembers recovered her body by winching themselves down from the helicopter and pulling the mother out of the ravine by air.
